Job description
**Job Title: Home Health Field RN**
The Home Health Field RN is responsible for delivering thorough nursing care, conducting patient assessments, and developing individualized treatment plans within the home environment. They work in partnership with interdisciplinary teams to maintain seamless care and provide education to patients and their families regarding health management. Applicants are required to possess a current RN license, a valid driver’s license, and CPR certification. A minimum of two years’ experience in home health is desirable, and candidates should demonstrate expertise in clinical documentation.
