The Youth Transition Specialist is responsible for overseeing a multifaceted caseload of young individuals, aiding their shift from secure environments to community-oriented placements. This role involves coordinating vital services such as educational opportunities, job readiness, and treatment programs, while acting as the key point of contact between families and multiple state agencies. Applicants should hold either a post-secondary degree in social work or have accrued 60 college credits along with two years of relevant experience in working with at-risk youth. A valid driver’s license is necessary, along with the capability to successfully complete a thorough background check and drug test.
