The provider will facilitate holistic behavioral health and substance abuse services within a multidisciplinary primary care team. Responsibilities include conducting assessments, developing treatment plans, and providing ongoing consultation to medical staff to improve patient outcomes. Candidates must hold a Master’s degree and possess a valid LCSW, LCMHC, or LMFT license in North Carolina. A minimum of two years of experience in a public health setting is required.
