**Job Title: Registered Nurse**
The Registered Nurse will focus on delivering tailored health coaching and managing chronic diseases for patients with cardiometabolic disorders through telehealth services. Key responsibilities involve assessing symptoms, educating patients, coordinating care with healthcare providers, and ensuring thorough clinical documentation is upheld. Applicants must possess a Bachelor of Science in Nursing (BSN) and an active Registered Nurse license, in addition to having a minimum of 3 years of hands-on patient care experience. A Certified Diabetes Care and Education Specialist (CDCES) certification is mandatory or should be actively pursued.
