Job Summary
The Care Manager, LPN plays a critical role in delivering high-quality healthcare support to patients in the community. This position requires a proactive approach to managing patient care by collaborating with multidisciplinary teams, providing care coordination, and ensuring that patients receive appropriate health services.
Responsibilities
- Conduct patient assessments to identify healthcare needs and develop care plans.
- Coordinate with healthcare providers and community resources to facilitate patient care.
- Monitor patient progress and modify care plans as necessary.
- Educate patients and their families on medical conditions and treatment options.
- Document all interactions and maintain accurate records per compliance guidelines.
Qualifications
- Licensed Practical Nurse (LPN) certification is required.
- Experience in care management or a similar role is preferred.
- Strong communication and interpersonal skills to engage with patients and healthcare teams.
- Ability to work independently and manage multiple priorities effectively.
- Knowledge of healthcare regulations and compliance standards.
