Provide direct care to assisted living or memory care residents, administer medications, conduct assessments, monitor changes in condition, and document care. Maintain individualized care plans, participate in interdisciplinary meetings, monitor vital signs and other measures, and support and train direct care staff while promoting fall prevention. Requires a high school diploma or GED, an active Licensed Practical Nurse license in the applicable state, current CPR certification, and at least two years of related experience.
