**Job Title: Case Manager**
The Case Manager is responsible for overseeing patient care plans and ensuring they adhere to regulatory standards while coordinating the processes of admissions, discharges, and transfers. Additionally, the role involves supervising field staff, delivering patient education, and ensuring accurate clinical documentation is maintained. Applicants must possess an active RN license in Georgia and current CPR/First Aid certification. A minimum of one year of clinical nursing experience is required, along with the physical capability to perform tasks such as operating a Hoyer lift.
