Job Summary
The LVN Sub Acute Per-Diem Days position is designed for a compassionate and skilled Licensed Vocational Nurse to provide high-quality care in a sub-acute setting. This role focuses on delivering essential medical and personal support to patients, ensuring their comfort and safety during their rehabilitation process.
Responsibilities
- Deliver nursing care to patients in a sub-acute environment, tailoring services to individual needs.
- Assist with daily living activities and ensure a safe, comfortable environment.
- Administer medications and treatments as prescribed by healthcare professionals.
- Monitor patient conditions and report significant changes to the healthcare team.
- Educate patients and their families regarding care plans and health improvements.
- Collaborate with other healthcare staff to provide holistic care.
Qualifications
- Current and valid LVN license in the state.
- Experience in a sub-acute or similar healthcare environment preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic knowledge of patient care protocols and emergency procedures.
