Provide personalized, one-on-one physical therapy interventions to patients in their homes to improve strength, mobility, and independence. Collaborate with an interdisciplinary team to develop treatment plans and maintain accurate clinical documentation. Requires a degree from an accredited Physical Therapy program and a current Iowa Physical Therapist license. Candidates must have at least one year of clinical experience and a valid driver’s license for home visits.
