Job Title: Social Worker (LCSW)
In this role, you will evaluate, advise, and guide patients with complex medical needs by creating personalized treatment plans. You will assist in their adaptation to disability, connect them with resources, and promote behavior change within their home and community environments. This position requires a Master’s degree in Social Work and active LCSW licensure in the state. Ideal candidates should demonstrate strong skills in planning, organization, written communication, and interpersonal communication.
