Job Summary
The Linkage to Care Specialist plays a crucial role in ensuring that individuals who have recently been diagnosed as HIV-positive, as well as those at risk, are effectively connected to and maintained in both medical and mental health services. This position involves not only providing continuous counseling but also coordinating appointments and performing field investigations to address and eliminate barriers that may hinder care access.
Responsibilities
- Assist newly diagnosed HIV-positive and at-risk patients in accessing medical and mental health services.
- Provide ongoing support and counseling to patients to enhance their retention in care.
- Organize and coordinate appointments to facilitate patient access to necessary services.
- Conduct field investigations to identify and address barriers to care.
Qualifications
- A minimum of 2 years of experience in HIV/STD prevention.
- Strong understanding of LGBTQ health issues.
- Excellent communication skills, both verbal and written.
- Proficient in using computer systems for documentation and coordination purposes.
- Ability to secure CPR and HIV counselor certifications within the first 60 days of employment.
