**RN Case Manager**
The RN Case Manager is responsible for orchestrating and managing all facets of patient care in a home setting while guiding a diverse team of healthcare professionals. Key duties include formulating comprehensive care plans, conducting physical evaluations, overseeing medication administration, and providing education to families regarding patient care requirements. Applicants must possess an active state license as a Registered Nurse, a current driver’s license, and certification in CPR. A minimum of two years of clinical nursing experience is essential, with a preference for candidates who have at least one year of full-time experience in hospice care.
