Job Summary
The Post-Acute Care Liaison is responsible for establishing and nurturing referral relationships with various healthcare providers to enhance admissions and overall patient census. Qualifications
- Active RN or LPN license is required.

- Strong interpersonal skills with a demonstrated ability to build and maintain relationships.
