Job description
As a Physical Therapist, you will perform thorough evaluations and manage OASIS Start of Care admissions for our home health clientele. Your role will involve creating and executing customized treatment plans while working closely with a multidisciplinary team to achieve the best possible outcomes for patients. Candidates must hold a valid Physical Therapist license in New Jersey and possess a minimum of one year of recent clinical experience. Experience in home health settings and familiarity with OASIS documentation are essential requirements for this position.
