Job Summary
The Advanced Practice Practitioner will work closely with physicians and the multidisciplinary team to provide comprehensive palliative and hospice care services. This role focuses on enhancing the quality of life for patients during their end-of-life journey through effective collaboration and care management.
Responsibilities
- Document patient medical histories accurately to inform care decisions.
- Manage and control symptoms to ensure patient comfort.
- Update and revise care plans regularly to maintain high standards of end-of-life care.
- Collaborate with healthcare professionals to deliver integrated palliative support to patients and families.
Qualifications
- Must be a Registered Nurse with an Advanced Practice Nurse license in Texas.
- Minimum of 3 years of nursing experience required.
- Proficient in Microsoft Office applications.
- Valid driver’s license with appropriate insurance coverage is necessary.
