Job Summary
The Advanced Practice Provider plays a vital role in delivering extensive primary care services within a Patient-Centered Health Home framework. This position involves the assessment and diagnosis of various health conditions, the management of both chronic and acute illnesses, and the collaboration with the healthcare team to promote optimal patient outcomes.
Responsibilities
- Provide comprehensive primary care medical services.
- Diagnose and treat illnesses, including managing chronic and acute conditions.
- Coordinate care efforts with a multidisciplinary health team.
- Ensure that patient-focused care outcomes are achieved.
Qualifications
- Valid California license as a Nurse Practitioner, Certified Nurse Midwife, or Physician Assistant.
- Completion of an accredited training program in a relevant field.
- Proficiency in advanced technical software, particularly the Microsoft Office suite.
