Job Summary
The Afterhours RN Care Manager plays a crucial role in delivering compassionate and top-notch hospice care during evening hours, weekends, and on-call periods. This position ensures that patients receive the support they need when it matters most.
Responsibilities
- Perform comprehensive patient assessments to evaluate their needs and conditions.
- Collaborate effectively with the interdisciplinary team to create and implement patient care plans.
- Ensure the accuracy of clinical documentation in accordance with standards and regulations.
Qualifications
- Must possess a valid RN license.
- Minimum of one year of experience in an acute care setting, oncology, long-term care, hospice, or home health is required.
- A valid driver’s license and reliable means of transportation are necessary for this role.
- Current CPR certification is mandatory.
