**Care Coach I (Hybrid)**
**Location:** Highlands County, FL (Sebring & Avon Park Area)
**Compensation:** $26.50 per hour
**Work Schedule:** Monday to Friday | 8:00 AM – 5:00 PM
**Employment Type:** Contract
**Travel Requirements:** 75-90% Local Travel
**Role Overview**
In the position of Care Coach I, you will act as a vital resource and advocate for members, assisting them in accessing necessary services and support to enhance their health and well-being.
**Key Responsibilities Include:**
– Performing thorough assessments to determine the healthcare and social needs of members
– Creating and overseeing personalized care plans
– Visiting members at their residences, assisted living facilities, long-term care facilities, and other locations
– Collaborating with healthcare providers, caregivers, and community organizations to coordinate care
– Identifying and addressing obstacles to accessing healthcare and wellness
– Connecting members and their families to community resources and support services
– Tracking member progress and revising care plans as necessary
– Accurately documenting all member interactions in electronic systems
– Encouraging positive health outcomes through education, communication, and continuous care coordination
**Qualifications Required:**
– Bachelor’s degree
– At least 2 years of experience in the fields of healthcare, social services, case management, care coordination, or similar areas
– Excellent communication and interpersonal abilities
– Proficient in Microsoft Word and Excel
– Familiarity with electronic documentation systems and healthcare software
– Valid driver’s license
– A reliable personal vehicle with minimum liability insurance coverage of **100/300/100**
– Capability to travel locally within the Highlands County region (75-90% of the working hours)
– Capability to undergo a Tuberculosis (TB) screening
– A genuine commitment to enhancing member experiences and healthcare outcomes
**Preferred Qualifications:**
– Master’s degree
– Experience serving Medicare and/or Medicaid populations
– Previous experience in case management or care coordination
– Familiarity with electronic health record systems or case management software
– Background in health coaching, wellness, or population health
– Understanding of community resources, healthcare organizations, and social service agencies
**Travel & Scheduling Details:**
– This is a hybrid role that entails both remote work and field visits
– Work hours are from Monday to Friday, 8:00 AM – 5:00 PM
– Travel will primarily occur within the **Highlands County area**, particularly in **Sebring and Avon Park**
– Regular visits to member homes, assisted living facilities, and long-term care environments
– Mileage reimbursement available for approved travel
