Job Summary
The Care Coordinator collaborates with a multidisciplinary team to promote the overall physical and mental health of clients. This role involves conducting assessments, making appropriate referrals, and coordinating care to meet clients’ needs effectively.
Responsibilities
- Conduct assessments to evaluate clients’ physical and mental health needs.
- Facilitate referrals to appropriate services and resources.
- Coordinate care among healthcare professionals to ensure comprehensive service delivery.
- Maintain adherence to healthcare program compliance and ethical standards.
- Provide support in both office environments and client homes.
Qualifications
- A Bachelor’s degree in social work or a related human services field is required.
- Candidates must hold a valid Iowa Driver’s License.
