**Job Title: Care Manager**
The Care Manager is responsible for evaluating, planning, and coordinating care for TRICARE beneficiaries via telephone, focusing on enhancing self-care and clinical outcomes. Acting as the main liaison, they work closely with military treatment facilities and healthcare providers to develop comprehensive care plans. Applicants must possess a valid and unrestricted license as an LCSW, LPC, LMHC, LMSW, or Clinical Psychologist, along with a minimum of three years of clinical experience. U.S. citizenship is essential for obtaining government security clearance, and the candidate must be capable of securing a Certified Care Manager (CCM) credential.
