Job Title: Care Manager
The Care Manager offers individualized behavioral support services aimed at assisting participants in building independent living skills while also connecting them with vital medical and recovery resources. This role involves engaging in care planning, responding to crises, and contributing to multi-disciplinary team meetings to ensure the delivery of effective treatment for clients. Applicants should possess either a high school diploma, a bachelor’s degree, or a master’s degree in a human services-related discipline, with experience qualifications differing according to the level of education attained. Additionally, a valid driver’s license is necessary to comply with vehicle safety standards and to facilitate client transportation.
