The Care Manager is responsible for conducting assessments, developing plans, facilitating care, and advocating for patients’ holistic health requirements. This role involves working closely with clients and various healthcare professionals to ensure effective and economical healthcare results. Applicants should possess a Bachelor’s degree along with a California Registered Nurse license. Required qualifications include a minimum of two years of experience in an acute hospital setting, which must include at least one year in a supervisory position.
