Job Summary
The RN Care Manager is responsible for the planning, coordination, and management of care for patients with complex medical needs across their entire care continuum. This position ensures that patients receive optimal outcomes while effectively utilizing available resources by monitoring clinical and financial discrepancies within medical records.
Responsibilities
- Plan and coordinate care for complex patients.
- Manage patient care across various stages of treatment.
- Monitor medical records for any clinical or financial variances.
- Ensure optimal patient outcomes through effective resource utilization.
Qualifications
- Associate’s Degree in Nursing.
- Current California Registered Nurse License.
- Minimum of 2 years of experience in acute care settings.
- At least 1 year of experience in care management or utilization review.
