**Job Title: Case Manager**
The Case Manager plays a crucial role in assisting individuals facing homelessness by providing support and referrals aimed at fostering self-sufficiency and securing stable housing. Key responsibilities involve performing client assessments, developing tailored case plans, and ensuring precise data entry in the HMIS system. To qualify, candidates should hold an associate degree in a social or behavioral science discipline and possess a minimum of one year’s experience in social services. Additionally, applicants must have a valid Arizona driver’s license and be eligible to obtain a Level One Fingerprint Clearance Card.
