Job Summary
The Case Manager plays a vital role in coordinating and providing comprehensive care solutions to clients in need. This position is focused on assisting individuals through the complexities of services available to them, ensuring they receive the support necessary for their well-being and growth.
Responsibilities
- Evaluate client needs to develop personalized care plans.
- Collaborate with healthcare providers and community resources to facilitate access to services.
- Monitor client progress and adjust care plans as needed.
- Educate clients and their families about available resources and care processes.
- Maintain accurate documentation of client interactions and outcomes.
- Advocate for clients to ensure they receive appropriate services and support.
Qualifications
- Bachelor’s degree in social work, psychology, or a related field.
- Experience in case management or a similar role.
- Strong communication and interpersonal skills.
- Ability to work independently and manage multiple cases efficiently.
- Familiarity with community resources and healthcare systems.
- Certification in case management is preferred.
