The Case Manager facilitates admission intakes, develops personalized treatment plans, and coordinates discharge planning for individuals in behavioral health crisis. They work within an interdisciplinary team to monitor safety, provide referrals, and maintain accurate medical records. Candidates must hold a Bachelor’s degree in a behavioral health field and possess at least two years of relevant experience. A Qualified Mental Health Professional (QMHP) certification and the ability to pass state background checks are required.
