**Job Title: Case Manager**
The Case Manager plays a vital role in delivering behavioral health services by executing personalized service plans and linking clients to housing, medical care, and employment opportunities. Key responsibilities include maintaining precise documentation, conducting crisis intervention, and providing continuous advocacy to ensure client stability. Applicants must possess a High School Diploma or GED and at least one year of direct service experience. A valid driver’s license, auto insurance, and access to a personal vehicle are mandatory for community outreach activities.
