Job Summary
The Case Manager plays a vital role in overseeing the comprehensive case management process, which encompasses patient evaluations, planning services, and ongoing monitoring. This individual will spearhead transition of care initiatives and collaborate with a multidisciplinary team to make certain that patient objectives are achieved.
Responsibilities
- Conduct thorough assessments of patients to identify their needs.
- Develop and implement individualized service plans.
- Monitor patient progress and adapt care plans as necessary.
- Facilitate transitions of care to ensure continuity and quality of services.
- Engage with interdisciplinary teams for collaborative care delivery.
Qualifications
- Current LPN or RN license from the state of Oklahoma.
- A minimum of two years of clinical experience in healthcare settings.
- At least one year of experience in clinical case management.
- Active Basic Life Support (BLS) and Cardiopulmonary Resuscitation (CPR) certification.
