They assess patient psychosocial needs and collaborate with the interdisciplinary team to develop holistic, individualized care plans. Candidates must hold a Master’s degree in Social Work and be a Licensed Clinical Social Worker. Proficiency in discharge planning, hospital operations, and knowledge of third-party payment systems like Medicare/Medicaid is required.
