The Licensed Clinical Social Worker coordinates and implements post-discharge plans while facilitating referrals to community resources. They assess patient psychosocial needs and collaborate with the interdisciplinary team to develop holistic care plans. Candidates must hold a Master’s degree in Social Work and possess a current Licensed Clinical Social Worker (LCSW) credential. Proficiency in discharge planning, community resource navigation, and hospital operations is required.
