Job Summary
The LVN/LPN is responsible for delivering exceptional nursing care to patients within their residences. This role emphasizes health assessments, medication administration, and wound care management. The LVN/LPN actively works alongside the healthcare team to design and execute personalized care plans that enhance patient autonomy and facilitate healing.
Responsibilities
- Conduct comprehensive health assessments for patients in home settings.
- Administer prescribed medications accurately and safely.
- Perform wound care and manage other nursing tasks as needed.
- Collaborate with interdisciplinary healthcare teams to create tailored care strategies.
- Support patient recovery and promote independence through effective care plans.
Qualifications
- Current LVN license.
- Valid driver’s license and auto insurance.
- Certification in CPR.
- Experience with HomeCare HomeBase is an advantage.
