Job Summary
The Home Health Occupational Therapist (OT) PRN/PT position is designed for a skilled professional who is committed to enhancing the quality of life for individuals in need of therapeutic services. The OT will deliver essential occupational therapy to patients within their own residences, focusing on rehabilitation and skill enhancement.
Responsibilities
- Conduct comprehensive evaluations of patients to determine their therapy needs.
- Develop tailored treatment plans appropriate for each patient’s condition and goals.
- Implement therapeutic interventions aimed at improving daily living skills and overall functionality.
- Monitor and document patient progress while updating care plans as necessary.
- Collaborate with other healthcare professionals to coordinate holistic patient care.
- Educate patients and their families on therapy processes and home exercises.
Qualifications
- Valid state licensure as an Occupational Therapist.
- Experience in a home health or rehabilitation setting is highly preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage a diverse patient caseload.
- Basic life support (BLS) certification is often required.
