**Job Title: Occupational Therapist**
Conduct thorough evaluations in occupational therapy and create tailored care plans for home-based patients. Work collaboratively with a multidisciplinary team to track patient progress and deliver specialized therapeutic services. Applicants must hold a valid Occupational Therapist license for the State of Ohio. A minimum of 1-2 years of experience in a healthcare environment is essential, along with a valid driver’s license and reliable transportation.
