Provide expert, patient-focused nursing care within a home health environment while working alongside a multidisciplinary team. Conduct comprehensive patient evaluations, oversee medication management, and instruct patients and their families on managing diseases and understanding treatment strategies. This position requires an active Registered Nurse (RN) license and a minimum of one year of clinical nursing experience. Applicants must have a valid driver’s license and the capacity to travel to patients’ residences.
