Job Summary
This position involves traveling to various healthcare settings, such as hospitals, nursing facilities, and patients’ homes, to evaluate patients and provide education to families regarding hospice care services. The primary focus is to assess eligibility for hospice care and facilitate a smooth transition for patients who choose this route.
Responsibilities
- Conduct assessments of patients in diverse care environments including hospitals and homes.
- Educate families about the available hospice care services.
- Determine patient eligibility for hospice care based on specific criteria.
- Ensure a seamless transition for patients opting for hospice services.
- Travel within the designated service area to meet with patients and families.
Qualifications
- Possession of a valid and current RN license.
- Reliable transportation to facilitate travel in the assigned service area.
- Experience working in a physician’s office, hospital, long-term care, or assisted living facility.
