Provide direct personal care and emotional support to terminally ill patients in their homes or care facilities. Maintain accurate documentation of patient visits and participate in team quality improvement activities. Requires an active CNA license, current CPR certification, and a valid driver’s license with a reliable vehicle. Candidates must have at least six months of direct patient care experience.
