Provide skilled hospice nursing care to patients in home, assisted living, and facility settings, including monitoring conditions, administering medications and treatments, and documenting and reporting changes. Collaborate with the registered nurse and interdisciplinary team, support patients’ families, participate in on-call duties and bereavement activities, and assist with death pronouncements where permitted. Applicants must graduate from an accredited practical nursing program, hold a current LPN/LVN license in the state of employment, and have CPR certification, a valid driver’s license, and current automobile insurance. At least two years of LPN experience in hospice, oncology, long-term care, or home health is required; three to five years is preferred, along with hospice care knowledge, strong assessment and communication skills, and familiarity with relevant regulatory standards.
