Job Summary
The Nurse Practitioner plays a crucial role in delivering extensive hospice and palliative care. This includes conducting patient evaluations, managing symptoms and medications, performing necessary hospice Face-to-Face encounters, and formulating tailored care plans. The practitioner will work closely with physicians, patients, families, and interdisciplinary teams to ensure effective communication and compliance with documentation while facilitating care transitions and assisting in end-of-life decision-making.
Responsibilities
- Perform comprehensive patient assessments to identify needs and symptoms.
- Manage medication regimens and symptom relief for patients.
- Conduct required hospice Face-to-Face encounters.
- Develop individualized care plans for patients in palliative and hospice settings.
- Coordinate with healthcare professionals, patients, and families to ensure holistic care.
- Maintain accurate documentation that adheres to guidelines.
- Support patients and families during transitions in care and end-of-life planning.
Qualifications
- Completion of an accredited Nurse Practitioner program with an MSN or higher degree.
- Current credentials as a Registered Nurse and Nurse Practitioner.
- National NP certification and applicable practice authority.
- Prescribing credentials and certification in CPR.
- Preferred two years of experience as a Nurse Practitioner.
- Experience in serious-illness care is advantageous.
- Strong clinical skills complemented by effective communication abilities.
- In-depth knowledge of hospice regulations and documentation requirements.
