Job Summary
The role entails providing expert nursing care to hospice patients, including thorough assessments, symptom management, and the formulation and continual refinement of personalized care plans. The position emphasizes the importance of education and support for both patients and their caregivers, as well as effective collaboration within interdisciplinary teams to ensure top-quality care.
Responsibilities
- Assess hospice patients and deliver proficient nursing care.
- Manage patient symptoms and develop/uphold individualized care plans.
- Educate and support patients and their caregivers to enhance understanding and comfort.
- Coordinate with interdisciplinary teams to provide comprehensive care.
- Communicate any changes in patient conditions promptly.
- Maintain accurate and timely clinical records to ensure continuity of care.
Qualifications
- Must possess an active Registered Nurse license in the relevant state.
- Hold current CPR certification.
- Have a valid driver’s license and automobile insurance.
- A minimum of one year of experience as an RN required; experience in hospice or palliative care is desirable.
- Strong assessment, communication, documentation skills, and knowledge of hospice care principles.
