Job Summary
The Hospice RN On-Call plays a crucial role in delivering after-hours clinical assistance, focusing on evaluating patient requirements and facilitating appropriate care. This position necessitates the assessment of patient conditions, which may be conducted through remote communication as well as direct field visits.
Responsibilities
- Triage patient needs for effective care management.
- Conduct assessments of patient conditions via various communication methods.
- Perform in-person field visits as necessary.
- Coordinate care to ensure patients receive appropriate support.
Qualifications
- Current Registered Nurse (RN) license.
- Valid CPR certification.
- Possession of a valid driver’s license and dependable transportation.
- At least 1 year of nursing experience in hospice, home health, or acute care settings.
