**Job Title: Lay Health Educator**
The Lay Health Educator plays a vital role in supporting health education initiatives aimed at patients with chronic conditions, including Diabetes and Metabolic Syndrome. Key duties entail conducting one-on-one appointments, facilitating educational classes, and ensuring accurate documentation of patient records. Ideal candidates will possess prior experience as a Medical Assistant and demonstrate bilingual proficiency in both English and Spanish. Effective communication abilities and knowledge of Windows-based software are essential for this position.
