Job description
The Licensed Clinical Social Worker (LCSW) will be responsible for evaluating, advising, and guiding patients with complex medical needs through the creation of personalized treatment plans. This role involves assisting patients in adapting to disabilities, connecting them with necessary resources, and applying behavior modification techniques both at home and within the community. Applicants must have a Master’s degree in Social Work and hold a current LCSW license issued by the state. Strong skills in planning, organization, written communication, and interpersonal communication are essential to successfully work alongside interdisciplinary teams and families.
