Job description
Evaluate, advise, and guide patients facing intricate medical conditions by creating personalized treatment strategies. Support patients in adapting to disabilities, connecting them with resources, and facilitating behavior changes in their home and community environments. A Master’s degree in Social Work and state licensure as an LCSW are mandatory. Applicants should demonstrate strong planning, organizational, writing, and communication abilities.
