Job Summary
The Medical Social Worker plays a critical role in delivering psychosocial assistance, conducting assessments, and managing care coordination for patients and their families in home environments. This position necessitates collaboration with a multidisciplinary team to tackle emotional, social, and financial issues, ensuring smooth transitions in care.
Responsibilities
- Provide psychosocial support to patients and families in their homes.
- Conduct assessments to identify emotional, social, and financial needs.
- Coordinate care in partnership with the interdisciplinary team.
- Facilitate effective transitions of care for patients.
Qualifications
- Master’s or Doctorate degree in Social Work.
- Active LMSW or LCSW license is required.
- Minimum of one year of social work experience in a healthcare environment.
- Valid driver’s license and CPR certification are necessary.
