Job Summary
The role involves providing both direct and indirect patient care while working alongside a team of healthcare professionals. The primary focus is on developing and executing individualized care plans tailored to meet each patient’s unique needs. Furthermore, the position includes the monitoring of patient health status and participation in discharge planning to guarantee smooth transitions for patients.
Responsibilities
- Deliver direct and indirect patient care in collaboration with an interdisciplinary team.
- Develop and implement personalized care plans.
- Monitor patient conditions to ensure optimal health outcomes.
- Participate in the discharge planning process for safe transitions.
Qualifications
- Active LPN license in Wisconsin or a compact state.
- Associate’s Degree in Nursing or a related field.
- Ability to obtain AHA Basic Life Support certification within 30 days of hire.
- Strong critical thinking abilities.
