The clinician will perform patient evaluations, identify and manage both acute and chronic health issues, and create tailored care strategies. Additionally, they will work alongside interdisciplinary teams to provide coordinated, patient-focused care while keeping precise medical documentation. Applicants should have a Master’s degree in Nursing or Physician Assistant Studies and hold an active license in New York. Preference will be given to those who are board certified in Family Medicine, Internal Medicine, or Primary Care, and candidates must also possess current BLS and ACLS certifications.
