**Job Title: Social Worker**
The role of the social worker involves delivering casework services, conducting psychosocial evaluations, and providing counseling to patients and their families in a home health environment. This position requires collaboration with a multidisciplinary team to create individualized care plans, assist with community resource referrals, and oversee discharge planning processes. A Master’s degree in Social Work is mandatory for applicants. Preferred candidates will have 1-2 years of experience in either home health or hospital settings, as well as a current driver’s license and CPR certification.
