**Job Title: Case Manager II**
The Case Manager II offers extensive support to individuals who have recently been released from incarceration. This role involves conducting needs assessments, creating tailored service plans, and linking clients to necessary resources. Additionally, the Case Manager II is responsible for crisis intervention, ensuring precise case documentation, and leading workshops on essential life skills to foster client independence. Applicants should have a Bachelor’s degree in a relevant discipline or a minimum of 4 years of direct experience in case management. A valid driver’s license, dependable transportation, and the ability to successfully complete background checks and health evaluations are mandatory.
