Job Summary
Palo Alto Foundation Medical Group is looking to fill a full-time position for a certified Nurse Practitioner or Physician Assistant specializing in Family Medicine. This role is based in San Carlos, California.
Responsibilities
- Provide comprehensive healthcare services to patients in a family medicine setting.
- Conduct assessments, formulate diagnoses, and develop treatment plans.
- Collaborate with healthcare teams to ensure quality care.
- Educate patients and their families about health management and preventive care.
- Document patient encounters and maintain accurate medical records.
- Participate in quality improvement and professional development activities.
Qualifications
- Must hold a current certification as a Nurse Practitioner or Physician Assistant.
- Possess a valid and unrestricted California license to practice.
- Exhibit strong clinical skills and a commitment to patient care.
- Demonstrate effective communication and interpersonal abilities.
- Prior experience in a family medicine environment is preferred.
