Job Summary
Geisinger is on the lookout for a dedicated Nurse Practitioner or Physician Assistant who is well-versed in family medicine to enhance our Primary Care team in Sunbury, Pennsylvania. This role is ideal for individuals committed to delivering high-quality patient care within a value-based care model.
Responsibilities
- Conduct detailed assessments and provide comprehensive care to patients.
- Manage appointment schedules, allocating 40 minutes for medically complex patients and 20 minutes for routine follow-ups.
- Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
- Maintain a focus on preventative health measures and chronic disease management.
- Document patient care plans and treatment progress accurately.
Qualifications
- Valid Registered Nurse (RN) license in Pennsylvania.
- Certification as a Nurse Practitioner or Physician Assistant.
- Experience in a primary care setting, particularly with family medicine.
- Strong communication skills and a passion for patient-centered care.
- A proven ability to work collaboratively in a team environment.
