Job Summary
The position involves working as a Nurse Practitioner or Physician Assistant on a PRN basis, specializing in in-home health assessments. Located in Bridgewater, Virginia, this role is essential in delivering comprehensive health evaluations to patients, either in their homes or at Skilled Nursing Facilities.
Responsibilities
- Conduct health assessments for patients in their residences or in Skilled Nursing Facilities.
- Provide treatment recommendations and health education.
- Facilitate physician referrals as necessary for further care.
- Assist in case management and coordination of patient care.
Qualifications
- Must possess a valid Nurse Practitioner or Physician Assistant license.
- Experience in home health assessments or related fields is preferred.
