Job Summary
The role of Nurse Practitioner or Physician Assistant (PRN) for In-Home Health Assessments is an exciting opportunity with Matrix Medical Network. This position focuses on delivering healthcare in various settings, including patients’ homes and Skilled Nursing Facilities, making a significant impact on patient wellbeing and outcomes.
Responsibilities
- Conduct comprehensive health assessments for patients in their residences or facilities.
- Provide health education, treatment recommendations, and referrals to physicians and case management services.
- Collaborate with multidisciplinary teams to ensure quality patient care.
- Document patient encounters and maintain accurate health records.
Qualifications
- Active certification as a Nurse Practitioner or Physician Assistant.
- Experience in a healthcare setting, preferably in home health or case management.
- Strong communication and interpersonal skills to interact with patients and healthcare teams.
- Ability to work independently and manage a flexible schedule.
